Due to the Paperwork Burden Reduction Act (PBRA) signed into law December 2025, applicable large employers are no longer required to mail Forms 1095-B or 1095-C to full-time employees.
If you would like to request a paper copy of your Form 1095-C (Health Coverage Tax Document), you may do so by any of the following methods. Paper copies are provided at no cost upon request.
