NSAA Pre-Participation Physical Examination Forms

Attention all 7-12 Parents!
NSAA adopted a new Parent/Student Consent form in October of 2022. This will be the new form. If you have already completed and returned your form, please complete the new attached form ASAP as athletes need to have this current form on file. Sorry for the inconvenience.
Physical Examinations will be required for all incoming 7th-12th grade students participating in sports during the 2025-2026 school year. Avera Medical Group, Elkhorn Valley Medicine, and local area chiropractors will be providing sport physicals. Physical forms and parent consent forms will be available at both medical clinics, the Jr-Sr High School Office, and O'Neill Public Schools Administrative Office located at 635 N 4th Street. Most insurance companies allow coverage for a sport physical. The cost of the physical is the responsibility of the individual/family and will not be paid by the School District. If you have any questions/concerns, please contact Mr. Nate Larsen or your coach. The Student-Parent Consent form will still need to be completed and returned to the office or the coach/sponsor prior to participating in practice. Physical examinations completed after May 1st will be accepted. Students will need to schedule an appointment with a clinic of their choice and return the proper paperwork to OHS in the fall.
